Optimizing Treatment and Improving Care Transitions in Chronic Heart Failure

The goal of the proposed activity titled: Optimizing Treatment and Improving Care Transitions in Chronic Heart Failure, is to provide education on how to improve patient outcomes through the integration of evidence-based recommendations, effective quality improvement strategies, improved patient transitions through the continuum of care, and the optimization of pharmacologic therapies for patients with chronic heart failure.


Upon completion of this activity, participants will be able to:

  • Review the epidemiology, pathophysiology, and diagnosis of chronic heart failure and how strategies to decrease hospital readmission rates and overall costs through better diagnosis and disease management impact patient outcomes
  • Discuss the safety, efficacy, and clinical application of the current and emerging therapeutics and the new recommendation for the management of chronic heart failure
  • Outline appropriate strategies to optimize pharmacological treatment of chronic heart failure


The target audiences for these activities include cardiologists, internists, primary care physicians, nurses and allied healthcare professionals.
